640cc silicone under muscle 2016. Upsized 800cc silicone 2024. Right is sitting lower at incision site & much fuller then right. Much better symmetry before in both size shape n location. What could be going on. Right side implant is not near the incision site. Could I be bottoming out on right side or could left side not be in the pocket or was muscle not fully released. Left is soft. Right is not as soft but not overly firm been on singular for 3 months as a precaution HELP

Yes, you have some bottoming out with these implants. Yes, you probably now will need mesh for better support. You do need to go down in size since your tissue is not supporting the weight of these heavy implants. Your implants are just too large for you.
Your tissue is not supporting the weight of these implants, and they are sagging down. This give less fullness at the top of your breast and less support to your nipple area which is collapsing inward. I suggest exchanging for smaller implants and placing them behind the muscle. Then, consider...
The lower side needs the lower pocket reshaped and perhaps a lift to even out your areolae. I would not suggest using high profile implants, but ones with a wider base. Naturally, your breasts sit somewhat far apart, and the bases cannot be moved.
