I had a revision capsulorrhaphy done in April after exchanging old implants 2 years ago. I made it known I had issues the first time around with not much tissue to my surgeon. He seemed to overlook that and I am still unhappy with my current look. My implants feel super bottom heavy and uncomfortable. I inquired about mesh and he disregarded it saying he did it with sutures? would mesh material have helped to achieve more fullness & less discomfort? implants are 495cc HP. thank you!!!

Your tissue is not supporting the weight of these implants, and they are sagging down. This give less fullness at the top of your breast and less support to your nipple area which is collapsing inward. I suggest exchanging for smaller implants and placing them behind the muscle. Then, consider...
The lower side needs the lower pocket reshaped and perhaps a lift to even out your areolae. I would not suggest using high profile implants, but ones with a wider base. Naturally, your breasts sit somewhat far apart, and the bases cannot be moved.
