Unilateral first time contracture after breast augmentation. What is the best option to treat capsular contracture?

There are a few variables to consider when planning a successful surgery to correct capsular contracture. Regardless of where your incisions are, any revision surgery should be performed via an inframammary incision to minimize implant contamination and to avoid any further injury to breast tissue. If the affected implant is in the subglandular position, it is better that both implants are switched to a subpectoral position. When removing the scar capsule, every effort should be made to remove as much as safely possible, preferably all of it, and this portion of the procedure should be done with only electrocautery dissection to limit bleeding and oozing of the raw tissue surfaces. I tend to reserve the use of biologic protein matrix material for repeated/recurrent capsular contracture, not for the first time patient. The reason being is recurrence of capsular contracture after a surgery I described here is very low, possibly as low as someone having breast augmentation for the first time, about 2-3%. There is no difference in the various brands of biologics.

My regimen for capsular contracture which I have posted on RS before is:
Complete capsulectomy, wash pocket with both hypochlorous acid and triple antibiotic betadine solution, strattice, drain and a new implant. I then send the capsules for PCR and if bacteria present (about 50%) we prescribe an extended antibiotic course. This regiment reduces my recurrence rate to under 5%.

There is good data on both Strattice and Galaflex in reducing capsular contracture rates. Alloderm as well but the human products like Alloderm are expensive
