At this time, there are two accepted surgical procedures for the removal of biopolymer injected into the buttocks. To effectively remove biopolymer buttock injections, there is ultrasound-guided, VASER buttock liposuction and direct buttock excision. The ultra-sound guided, VASER liposuction removal procedure avoids the incision across the top of the buttocks. During the ultra-sound guided, VASER liposuction removal procedure, tumescent fluid is injected into the area. An intra-operative ultrasound is used to identify the biopolymer. A VASER probe is used to melt the scar tissue around the biopolymer. Then, I use a proprietary liposuction cannula to remove the scar tissue around the biopolymer as well as the biopolymer itself. The ultra-sound guided, VASER liposuction removal procedure takes approximately 2 to 3 hours to complete depending on the amount of biopolymer. This procedure is performed under general anesthesia. Silicone removal can be performed surgically without the need for a large, unsightly horizontal incision across the buttocks. I’ve developed a specialized technique utilizing a discreet “Y”-shaped incision, strategically placed within the gluteal crease. This approach extends only about an inch beyond the crease on either side, allowing for optimal access while keeping the incision primarily hidden within the natural contours of the body. I am a board-certified plastic and reconstructive surgeon with a specialized focus on silicone and biopolymer removal. Over the years, I have dedicated my practice to safely addressing complications arising from these materials. Recognizing the unique challenges they pose, I developed a two-stage surgical excision process and a third stage of reconstruction (often referred to as a reconstructive BBL).
