for a breast augmentation.  Round smooth silicone implants will feel softener than anatomic.  The size, profile, and shape of the
implant is based on your desired breast size/shape, your chest wall
measurements, and soft tissue quality.  This decision should
be based on a detailed discussion with equal input from both you
and your surgeon.  This entire surgery
can be performed with a small incision technique.  Make sure you
specifically look at before and after pictures of real patients who have
had this surgery performed by your surgeon and evaluate their results.   The most important aspect is to find a
surgeon you are comfortable with. I recommend that you seek consultation with a
qualified board-certified plastic surgeon who can evaluate you in person.

The key to a nice result in your case is making sure your implants fit your body appropriately.  Most importantly, the implant base width should match your chest base width for a natural result- if the implants are too large or too small, they will look less natural.  There is no perfect implant- each has its pros and cons.  A textured anatomical implant can look like a softer smooth round implant, but it will have a more form stable gel, more stability in the pocket (less risk of malposition / migration), and a lower risk for capsular contracture.  Cons include a (low) risk of rotation or late seroma (

Hard to answer this just based upon your photos and without an in person exam and specific breast measurements (and not just your height/weight). Remember also that there is no direct correlation between implant size (cc's) and cup size. Best option is to choose a board certified plastic surgeon who actually takes the time to measure you and examine your breast pocket. Sizers will only give you a rough estimate. The goal of breast augmentation is to optimally fill your breast pocket, not to overstuff or understuff it. At my practice, we utilize a series of 5 breast measurements that help me determine the ideal implant size, which in turn helps me deliver a 24 hour recovery. Do your research and choose a surgeon who takes the time to sit down with you and discuss your goals/expectations to determine the best size for you. Good luck!!

Thank you for your photos and questions. In general a round implant provides more upper breast fullness than an anatomic implant. The round implants are usually softer but there are some enhanced cohesive round implants that have been produced with a firmer gel like the anatomic implants. There are many other factors involved in breast implant selection so please see an experienced board certified plastic surgeon to select the implants that will best suit your goals.

Make appointments with at least a couple of experienced breast surgeons in your area that use both implants and ask if you can touch the implants. You will likely find the smooth round implant to be softer and more natural feeling. Hope this helps,good luck!

There have recently been some studies that show even plastic surgery professionals cannot tell the difference between round and anatomic implants (see link below). That being said, you would want to work with a board certified plastic surgeon and his/her staff to determine the best size for you. 

Careful selection of size and fit of the implant is likely more important than shape in your case. Studies have shown that especially with smaller implants round or teardrop implants look about the same. And the round implants will feel softer.

Based on your photographs, I think that you are starting at a good place and should have a very nice outcome with breast augmentation surgery.  Ultimately, careful communication of your goals (in my practice I prefer the use of goal pictures, direct examination/communication in front of a full-length mirror, and computer imaging) will be critical.  Personally, I think that for most patients considering breast augmentation surgery, careful selection of plastic surgeon and careful communication of goals, far outweigh the specific type of breast implant utilized (when it comes to the success of the breast augmentation procedure). Anatomical breast implants were designed to better simulate the natural breast shape; the problem with these implants is that if they shift or rotate, the entire breast will change shape as well. This change in breast shape may require further surgery to correct. Round implants, on the other hand, can shift or rotate without changing the shape of the breasts. ***Overall, I find that beautiful outcomes can be achieved with the use of round saline or silicone gel breast implants for the vast majority of patients.
 Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery ( regarding breast implant size/profile selection) is:
1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work.
2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. 
In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. For example, I have found that the use of know words such as “natural” or "B cup” etc means different things to different people and therefore prove unhelpful.
Also, as you know, cup size varies depending on him who makes the bra; therefore, discussing desired cup size may also be inaccurate. Again, the use of computer imaging has been very helpful during the communication process, in our practice.
3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery, after the use of temporary intraoperative sizers.

Many studies are now out there to suggest that there is no visible difference between these 2 implant styles in a natural looking result and profile selection. Because of increased costs of anatomials and increased risks involved, most would suggest a conservative profile implant in smooth round silicone under the muscle.
