Good morning,I don't put a lot of faith in fat transfers- they don't last and can get very lumpy bumpy, but what I would recommend that does work is an areola reduction- this will make your areolas more proportioned to your chest, and will also reduce your puffy nipple appearance.
No, fat will be unable to increase you much in size nor can reshape the breasts well. Implants can reshape and maintain the new shape and will give you the size you want. Fat transfer could also make your areolae worse since breast tissue is mainly present inside your areolae.

Your breasts do not appear tuberous to me. I would recommend reducing and lifting the larger side then lifting the smaller side to match.
