Butt implants placed within the muscle will appear and feel more natural.Kenneth Hughes, MDLos Angeles, CA
I have had the pleasure of operating on many very petite patients for buttock implants. The technique and most important factor always remains the same - the precise placement of the implants under the gluteus maximus muscle. This provides for not only an extra layer of padding covering and concealing the implant but also holds the implants in stable position over the long run. Other important factors in creating the perfect derriere with implants is selecting the right size AND shape implant. I have found the best way to assure this is to simply measure the actual dimensions of the patient’s buttock cheeks. With these anatomical measurements, I can select an implant that not only significantly augments the buttock but also supplies the correct 3-dimensional enhancement to look as real as possible. Therefore the only way for you to know what implant works best is to consult with a board certified plastic surgeon specializing in this procedure and have him/her take your measurements before selecting an implant and proceeding with surgery. Glad to help.
It is very difficult to determine the exact size and shape implant you will require to best match your ideal butt image without an examination by a board certified plastic surgeon. Not just any board certified plastic surgeon, but one with many years of frequently performing breast augmentation surgery including different approaches, techniques and implant choices. This is because several measurements not to mention your breast characteristics are needed to determine the optimal implant size to obtain your goals. Without knowing these dimensions it would be difficult to make this determination. For example, the existing base width of your buttock will determine, in many cases, the maximal volume per implant profile that you can accommodate. Liposuction of your lower back and fat transfer is also helpful as an adjunct to implants. Using “want to be” photos however are useful if simply provided to the surgeon.
This is one of the main reasons why i abandoned this technique. If your implant is placed within the muscle or submuscular, this wont happen.I,like a few other colleagues offer online consultations where you send us yourphotos and we can estimate the cost. Find a Board Certified Plastic Surgeon anddiscuss what will be the proper treatment to achieve the result you desires.Regards,Dr. Luis Redondo
Dear Steph,If you are that skinny and have that large of an implant placed above the muscle, then you can expect the same look. Be sure that you are aware of all of the risks of placing the implants in or below the muscle as well before you proceed. Good luck!
If the implants were placed above the muscle then they would look like the picture in a very thin person. If they were placed intramuscularly they would not.
It looks like those implants were placed subcutaneously in a very thin person. Best in this case to probably place them under the muscle.
The picture posted shows what a buttock implant should NOT look like. Large round implant in the subcutaneous plane!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
There are several incisions that have been used, but an incision in the crease between the buttocks is common.Kenneth Hughes, MDLos Angeles, CA

Regarding buttock augmentation, let me start off by saying that I currently see at least 1 - 2 patients per week that failed "brazilian buttock lift" (i.e. the fat melted away after 10 - 12 months) and are now seeking a more reliable and permanent option (i.e. buttock implants). One of the most...
