Hi all, I’m 33 female. I’ve recently noticed in the year my lower face is aging terrible. I do get DAO Botox which sometimes when I look in the mirror I think I’d Great but just not enough,my Marionette lines have definitely approved. My main issue would be the corners of the mouth, they ato sy the lea. I am researching deep plane facelifts, do you think would be a benefit or to wait a few years? Also what would help outside of surgery, thanks!

Although it is always possible to see facial changes even at an earlier stage in life, it is a mild problem in your situation, and it would be very hard to justify a Deep Plane Facelift intervention.

Consistent use of sunblock, irrespective of the weather, helps to protect the collagen fibres in the skin and delays the appearance of laxity or skin changes generated by ultraviolet light.

The treatments that you have already seem adequate for the purpose.

At 33, it is completely understandable to feel frustrated when you start noticing changes in your lower face, but looking at your photo, your skin elasticity and tissue support are still incredibly youthful. Jump-starting into a surgical deep plane facelift right now would be highly premature and offer very little benefit, as there simply isn't enough structural laxity or severe sagging to lift. Since your main concern is the downturned corners of the mouth and emerging marionette shadows, my suggestion would be to stick with non-surgical strategies for the next several years; continuing your DAO Botox is excellent for releasing the downward pull, but you can beautifully complement it with subtle dermal fillers or a touch of skin-tightening treatments (like Morpheus8 or Sofwave) to stimulate localized collagen. A tiny drop of filler placed strategically at the oral commissures and along the marionette lines will give you that instant structural support and "lift" you're looking for, keeping things refreshed and natural while allowing you to push any thought of surgery far into the future.

The photo is not good enough to give a good evaluation. Based on the single photo of your lower face, I see very early marionette lines around your mouth and a slight downturn of the corners. Otherwise your skin looks smooth, with no real jowls and no platysmal banding. You are not ready for a deep plane facelift. A deep plane on you would be equivalent to using a sledgehammer to crack a nut.

For the image you are hoping to achieve, the treatments I would do are office-based and staged over time. DAO Botox you already get, fillers in the marionette grooves, a touch of cheek volume from above to support the corners of your mouth, and an oral commissure lift if the downward turn is what really bothers you. The oral commissure lift is a small in-office procedure that elevates the corner of the mouth using a tiny excision at the lateral lip. Radiofrequency microneedling, such as Morpheus8 over the next year, will build collagen and tighten the lower face with essentially no visible scars or downtime.

On the surgical front, the ideal facelift window for most patients opens up in their mid-40s. So you are at least ten years away from needing one.
