I had a breast lift and augmentation exactly one year ago. Per my plastic surgeon, I was a “difficult” case. I had breastfed and had an 80lb weight loss. He also said I had tubular breasts. I’ve included before & after pictures and what they look like now. My right breast is heavier and fuller towards the bottom it also feels “saggier” and like there’s more skin on that side. I have nipple assymetry. I am supposed to have an in office procedure next week. My surgeon stated he believes he can remove some skin in the area I have circled and fix the nipple asymmetry that way. My question is will this be a long term fix? Will tightening up the skin near the crease make this breast not feel so heavy and “saggy”? Do I need pocket tightening instead?

I would suggest going down smaller and tightening the lateral pockets with mesh to help prevent the migration. You will need to avoid heavy lifting and heavy upper arm workouts for 3 months.
You do need to go down in height and possibly some in size with your implants. Also, your sagging breast tissue needs to be lifted. Your side view shows this the best.
