Hello! How can I get rid of festoons? (Photos)

Question

I don't know if that are festoons but ai think they are. I look tired with them and I don't know how to get rid of them. I would also like to know if there is any natural way, without fillers, surgery... thank you :) :(

I agree with you that you seem to have malar festoons. These can fluctuate on a daily basis based on the amount of fluid retention. The best possible treatment is CO2 laser to shrink and tighten the overlying skin. Home remedies can include low salt diet, avoid sun exposure or smoking and maybe applying pressure with tape overnight could decrease the problem. Good luck

There are many different causes of this issue and thus there are a few different  ways we treat this issue. If your skin has sun damage then we can often make a difference with laser.If the issue is mainly volume loss we can use fat transfer or fillers.If the skin is in excess a lower lid blepharoplasty surgery is an option.Sometimes the midface has sagged or become aged and a facelift is the best option. All of this would be determined after a careful physical exam and facial analysis. Hope this helps.Best of luck,DrC

Dear Little Girl,It appears that you do have lower eyelid festoons / malar mounds. These can be present in some early in life and others after years of sun exposure. They can wax and wane looking better or worse at different times. The best way to improve this condition is to injure the area and have the body heal the injury resulting in new collagen and elastic fibers and therefore tighter skin.I've had success using lasers to tighten and improve festoons. I'll leave a weblink below. I hope this was helpful.

This can be improved with lasers, well-placed fillers, and microneedling/prp. I recommend getting a formal evaluation with a cosmetic dermatologist. Surgical can sometimes worsen this area.  Best, Dr. Emer

There are other options to help improve this area if you are not interested in fillers or surgery.  You can consider laser treatments, chemical peels, and/or microneedling (Skin Pen) to help improve overall skin tone and texture.

Thanks for your question.  There are several non-surgical ways to improve the appearance of your lower eyelids.  This can be done through a combination of fillers and energy-based treatments such as ultrasound or laser.  For you specifically, I would recommend this approach compared to a lower eyelid lift, as that is typically a better option for patients who have more dramatic changes of their lower eyelid and upper cheek. Thank you for your question. From the photos provided it appears you have prominence of what I call periorbital hollows. These occur when the natural attachments from the underlying facial skeleton to the skin become more prominent creating a tethering effect. In between these ligaments fluid can sometime accumulate leading to a true malar bag (festoon), or normal soft tissues protruding forward can create shadowing leading to a tired appearance. These are common as we age but can also be present at a young age based on your anatomy and a genetic predisposition.  No one patient is the same and therefore no treatment plan is the same for every patient.  In your case I believe appropriately placed fillers would likely result in a good outcome.  Specifically fillers in the tear tough area (the shadowed area closer to your nose and eyelid) would help diminish the shadowing. Fillers could also be placed along the cheek depression to diminish the prominent fold. Surgery can be performed and fat transposed to fill in these hollows but only by someone experienced in lower eyelid surgery. Laser therapies can help with the texture and tone of the skin but are probably not going to work completely. Best of luck!
