I had a bilateral nipple sparing mastectomy with tissue expanders that were exchanged three weeks ago for Mentor round moderate profile silicone implants submuscular and am already noticing rippling on the medial side of both implants. What can be done to correct this as I am very thin and have no fat to graft.

Answer: Implant rippling at 3 weeks post op. There are different surgical techniques that can be utilized to sometimes improve this complication. These techniques vary from changing the type of implant, changing the position the implant is placed (which could include placing the implant under the muscle if it was originally placed above the muscle), or sometimes even using grafts which can camouflage the edge of the implant.

Answer: Post breast reconstruction rippling with a gel implant. If there is inadequate soft tissue coverage any implant maybe visible and palpable. Textured implants can produce traction rippling. Smooth shelled implant may decrease rippling (however in a thin patient there can still be rippling). Rippling may be do to a pocket that is too tight that squeezes the implant or too large that does not compress the implant. A smooth shelled implant can be massaged too stretch the tight pocket. Alloderm is often used in breast reconstruction to camouflage the implant. This is probably the best option in a thin patient. Fat grafting (large volumes usually not necessary) can be harvested from multiple sites in a thin patient and placed to soften the implant edge. Other reconstructive procedures such as abdominal muscle flaps likely were discussed in the consultation. 3 weeks is early to judge the final appearance. If a secondary procedure is considered the decision should wait until at least 6 months post op.
