I have a BBL scheduled for November. I have cellulite with dimples on my butt and thighs. I know BBL will not remove the cellulite. So should I have the Cellfina treatment first or wait until after I have the BBL?
Answer: Cellfina before BBL Belle, thank you for this excellent question. I do not believe that there is good scientific evidence to support which approach is best here. My practice is to perform Cellfina prior to BBL. In patients with cellulite of the buttocks, after BBL dimples often remain and may even become more exaggerated. The cellulite restricts skin expansion and limits the volume of fat that can be grafted. Surface contour irregularities remain or become exaggerated. Cellfina will release the buttocks dimples and relieve the skin tethering to deeper tissue. With a subsequent BBL the skin will then be free to expand smoothly and evenly and a larger volume of fat may be able to be grafted. I do not believe that the two procedures should be performed together. The Cellfina releases create cavities that will be filled with fat with a simultaneous BBL. These larger deposits of fat will be prone to more resorption from lack of revascularization and to nodule or cyst formation. Ideally BBL will be performed at least 3 to 6 months after Cellfina to allow for maturation and softening of internal scar tissue. Cellfina itself is the best treatment we have for cellulite but works best in combination with Sculptra you may need cellfina for cellulite, sculptra for collagen production, renuva fat injections or autologous fat injections and body treatments like Venus Legacy/Exilis Ultra, Cellutone/Zwave. Dimples can be improved with a combination of surgery, fillers, and/or lasers and microneedling/prp. A VASER HI DEF procedure can be done for more contouring and smoothing, if you're a candidate. First of all, it addresses the fact that the Brazilian Butt Lift (BBL) will not treat the cellulite in the butt and thighs. The BBL is a great treatment to enhance your buttock and overall shape, however it does not treat the superficial cellulite. There are several ways to address the cellulite for your butt and thighs. The options are surgeon specific and should be discussed with your Board-Certified Plastic Surgeon performing your Brazilian Butt Lift. The first option is to have the Cellfina done at the time of your Brazilian Butt Lift if your surgeon has that option available for you. This is easier logistically as your two procedures are combined in one. When choosing between doing the procedure before or after the BBL I would recommend doing the Cellfina prior to the Brazilian Butt Lift as it will release the bands and in my patients, it seems to allow for more fat transfer. However, if you do it prior, it needs to be several weeks prior to your Brazilian Butt Lift. With all of this said you are still a candidate for Cellfina after your Brazilian Butt Lift. I typically have patients wait at least six weeks after their BBL to undergo a Cellfina treatment. The versatility of being able to do the Cellfina in the office under local or in the operating room as part of your Brazilian Butt Lift gives patients a lot of options. I hope this helps. You can have Cellfina as a secondary procedure to improve any residual cellulite 3 months after the BBL.
