I would agree with your assessment. It looks like the skin is tightly attached to the chest muscle underneath. Usually, we try to leave a little cushion of fat under the skin in order to prevent such a problem. I would think that the skin could be separated from the underlying muscle and fat transferred into the area in order to add some volume there. I'm just not sure where they are going to find any fat on you to transfer.

Sometimes after gynecomastia surgery, there is adherence of the skin to the underlying muscle. It is a very delicate balance to preserve enough tissue between the two yet achieve the contour desired. Fat grafting and release may help.
