I'm having my 450 CC saline breast implants removed with capsulectomy & will also have a lift however my DR said looking at mammograms I'm about 90% implants so not much tissue. I was a B cup before implants but I'm now 10 pounds lighter then when I got them 24 years ago with older skin.I have been reading about autoaugmentation where they use the skin to build the breast, is that something that would work for me. I'm afraid to be flat & hearing so many FT Horror stories lately of fat necrosis.
Answer: AutoAugmentation after explant with capsulectomy? Auto-augmentation would only provide a fraction of the volume you would lose from explantation. If your preference is to not have implants, fat grafting would provide greater volume. But even fat grafting will not restore the 450 cc volume loss.

Answer: Staged Procedure Auto-augmentation uses your own breast tissue in a breast lift procedure. Fat grafting would be more appropriate for you. After initial grafting at the time of explantation, you might need a subsequent grafting procedure to achieve more volume. Keep in mind neither would give you the appearance you have now with breast implants.

Answer: Explantation Great question! Because your native tissue only makes up approximately 10% of the beasts, there really isn't too much auto augmentation that can be done. Basically instead of removing all the skin and tissue that you would normally do in a breast lift, only the top layer of the skin is removed and the remaining tissue is used to help provide volume. However, this is not nearly enough to provide any type of fullness or perkiness to the breasts. You could always try this first, and then possibly think about a smaller implant in the future, or you can consider fat transfer at the same time or later. This is why implants are still very popular because its the only way to provide that upper breast fullness and projection that many patients desire. Answer: Breasts implants I totally support your desire to have implants removed if you’ve had problems with them. When women mention auto augmentation they are usually thinking about fat, not usually skin. In either case I don’t think you can expect much volume so I advise against either method because it will not do much as far as providing volume.

Answer: AutoAugmentation after explant with capsulectomy? Thank you for your question and photo. Based on the information that you provided, you have a very thin rim of breast tissue around your implants (which make up 90% of your breast volume). I would advise you NOT to have a lift at the same time as your total capsulectomy and implant removal. Your risk of potential nipple necrosis is high. Why would you want to take that chance with an elective operation? I would do the explant and total capsulectomy, period. You could have a lift later ,if needed, or can have fat transfer into the breasts.I'm glad you are researching your options. As with any surgery, if it is done by an unqualified surgeon, horror stories are common.In your case, an auto augmentation will not help due to the lack of tissue that would be transferred superiorly. I'm glad that other surgeons use this technique, which I have been doing for my patients in the last 15 years. This does work in the right patient. Fat grafting works too in the right patient, and in your case, you will probably need more than one session to achieve a B/C cup.For your information, autoaugmentation of upper poles of the breasts and lipo 360 are not marketing terms. This might be considered "marketing" by the surgeons who do not have these powerful techniques in their armamentarium.See a board certified plastic surgeon experienced in this type of breast surgery and advanced body sculpting techniques to discuss the best long term options for you.
