Marionette lines develop for a number of different reasons. There is no single treatment which will provide optimal results in the treatment of this area. There is certainly a role for noninvasive skin tightening but there is also a role for injectable fillers.
Without seeing pictures it is hard to say which procedure will work best for you. Usually marionette lines are best treated with fillers. I would recommend going to see the nearest physician to evaluate you. Hope this helps.
I recommend fillers to soften the nasolabial folds themselves and also lift around the mouth and cheeks to build underlying anatomical support. A thorough history and examination of the entire face and neck is going to determine the choice of treatment for marionette lines. An obese patient with submental fat, folds, deep wrinkles, heavy jowls, and severe laxity of the skin along with marionette lines will not get the best result with either Ultherapy or Infini. If you have moderate global skin laxity, adequate facial volume, and minimal jowls then either Ultherapy or Infini can be used. Ultherapy is more precise since it is performed under direct visualization. Both of these procedures are painful but results can be excellent. Threads such as Novathreads or Instalift can help the marionette lines in selective individuals.
Ultherapy and Infini both can tighten areas with skin laxity. Infini is a radiofrequency, microneedling device that delivers results for skin tightening, scar revision, and wrinkle reduction. The treatment can tighten lower parts of the face. Fillers can also reduce the appearance of lines around the lines.
Ultherapy is designed to tone and tighten deeper tissue layers of the face, neck and beyond. It does so better than any other non-surgical treatment I know. It is not the appropriate treatment for Marionette lines. Fillers (and maybe a facelift) will address them effectively.
These are tough lines to treat no matter what you do. Ulthera done in the lower half of the face will help to tighten the marionettes. I like ulthera mostly for helping to tighten the jowls and under the chin. Infini I like for people who have lip lines and lots of fine lines and wrinkles around the mouth. The other good option for marionettes lines is filler.
Ultherapy uses focused ultrasound for noninvasive skin lifting, and whether it will improve marionette lines is a determination that can only be made by in-person evaluation. Infini is one of many radiofrequency devices such as Thermage and as a category they are not as precisely focused as Ultherapy. You may need fillers for the marionette lines. None of these is a true alternative to a surgical facelift.
Ultherapy will lift, tone and tighten the loose skin of the face. This may provide a slight improvement in the appearance of marionette lines. The main issue with marionette lines is a loss of volume and fillers work much better in replacing the lost volume. A combination of Ultherapy and Fillers would be best. Hope this helps.
Neither of these devices will work for the marionettes. Filler will help in most cases. If there is still a problem after proper filling, I'm now using PDO threads to improve this area. This is a technique from Korea and Europe. This has been a difficult area to fully correct in the past with just fillers. Skin tightening devices help only minimally.
