it is hard to tell for sure without an examination. Based on your photos, it looks like you would need surgery and it can probably be done under local anesthesia. A revision could be done and use only internal sutures. You will also need a permanent suture internally to help decrease the risk of stretching. the areolar tissue is very stretchy.

I'm sorry to hear of the issues you've had. From your photos it appears you can consider a revision of your areola reduction. There are no guarantees that stretching will not occur again. Your surgeon will examine you to determine if you are a high risk candidate for this.

If you dislike the size of your areolas a revision periareolar mastopexy will reduce the size of your areolas. The biggest risk is that this recurs. If you dislike just the pigmentation and starburst pattern shape that the areolas have and want the areolas to remain the same size, then you would benefit more from tattoo.
