I believe I would need a fat transfer to cheeks, (maybe) chin, and definitely a rhinoplasty. I've been told I have the beginning of jowling and would benefit from a lower facelift to help chin, jawline and neck. I assumed I would be fine with a buccal fat removal and maybe some facial liposuction but I'm not sure? My end goal is to have a facial balance similar to Shay Mitchell since I believe I also have a heart shaped face. I'm open to any opinions and would love some insight !!

Answer: Facial contour Unfortunately doing facial contouring isn’t like working with clay. Each individual person‘s facial look or individual characteristics is primarily determined by facial skeletal structure. It is the bone that creates individuality and helps us recognize each other including what people consider attractive, unattractive, masculine and feminine. Soft tissue coverage is very consistent from person to person and changes only with age. When young people want to change their facial appearance surgical approaches need to take primary issues into consideration and address the primary problem not do so with soft tissue manipulation. What you described as a simplistic approach that has a high likelihood of leaving you in regret of what you wish for. If you want to get a proper assessment in regards to facial bone structure then see board certified plastic surgeons who have cranial facial background training or oral surgeons. Facial liposuction needs to be done with great caution in order to avoid facial nerve injuries. Facial fat transfer is tricky business and once fat is grafted in place it’s very difficult to reverse the outcome. Many people come to regret having buccal fat pad removal if not immediately then typically later in life as with age facial volume loss becomes one of the major components of facial aging. Look for senior providers with many years of experience and consider having multiple consultations hopefully with each provider confirming the same recommendation. Best, Mats Hagstrom MD
