You first need to be seen by a board certified plastic surgeon so that he examines your neck and determines the location of the fat. If the fat under your chin is above the muscle (the platysma muscle) you would be a candidate for micro liposuction. There is no need for laser or any other energy based procedure. I perform the micro liposuction with a very small cannula and a syringe right in the office under local anesthesia. The recovery is the quickest with this technique. If however the examination shows that the fat is under the muscle, it would have to be removed through an incision under the chin directly. This can also be done under local anesthesia.

With significant neck sagging you would need a facelift / neck lift with muscle tightening (platysmaplasty). Neck liposuction is a great option for patients with good skin elasticity and excess fat in the neck and around the jawline. This will remove excess fat and stimulate the skin to contract over time which will improve angulation and definition along the jawline. I recommend seeing an expert for a consultation who can go over this procedure to determine if it will achieve your desired results and the cost of the procedure. I hope this helps!

Double chin is caused by excess of adipose tissue (fat) under the chin. To remove the double chin it is necessary to undergo a procedure that takes the fat away. Either lipo, microlipo or start lipo takes the fat away since they all include lipo. Smart lipo in addition of lipo tightens the skin with a laser. I only recommend smart lipo in patients with loose skin of the neck, usually in patients above 40 years of age. In my patients in their 30s straight forward chin lipo does a great job. 

Smartliposuction is a great way to perform submental liposuction because it can tighten the skin with the laser portion of the procedure and remove the excess volume with liposuction.  All forms of liposuction (smart, micro, vaser) have essentially the same downtime as all remove fat with a cannula.  A surgeon will need to assess the quality of your skin to give you the best answer.  Loose skin with no elasticity may need a neck lift vs, skin that has some "snap" which may respond well to liposuction alone.  

The photograph is rather limited, there appears to be extensive fatty deposits in the neck located above and below the platysma muscle. Liposuction can only remove the fatty deposits located above the platysma muscle, while  a surgical neck lift is required to remove the fatty deposits underneath the platysma muscle. Thank you for your question and posting an appropriate picture. You seem to be a good candidate for SmartLipo (laser) otherwise known as MPX, triplex, etc. Micro Lipo is done at the same time. It is a procedure done under local anesthesia with 3 to 5 days of downtime. Please meet a board certified plastic surgeon who has all these technologies to speak about the pros and cons of each. VASER (ultrasound) is best for men or where lipo was done before. You would need a consult for precise recommendations either in person or via Skype or FaceTime. 

 Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years.  From the photos, your chin is very weak as are the back portions of the jaw line and there is a very slight amount of excess fat under the chin.  When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin".  
 Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face.  Using the same incision, liposuction can be performed to reduce the fat and further shape the neck.  Excess skin, from below the chin, can also be removed through the same incision.  
 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty (requires extensive tissue dissection, bone cuts and placement of metal screws and plates to secure the cut segments of bone).  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.  In my opinion, you are a good candidate for chin implant surgery.  The jaw line can be augmented, with precise placement of a dermal filler, without masculinizing the jaw line. In contrast, reducing the slight amount of fat from under your chin alone, using any method, will create minimal change as it can't provide the forward chin projection needed to correct the weak chin.Hope this helps.
