Answer: Rhinoplasty Hi @qwert678. Thank you for sharing your photos. You are an excellent candidate for a rhinoplasty. There are several options for you that can be determined after examination. In my practice, I utilize piezosurgery. Piezosurgery is a more gentle technique that allows for greater precision. Less bruising and swelling is common with Piezosurgery. In a typical rhinoplasty, a hammer and chisel technique is performed leading to more inadvertent fractures. Best of Luck, Dr. Nima

Answer: Rhinoplasty Hello and thank you for your question. Although an exam in person is ideal, from your photographs alone it appears that you can benefit from a rhinoplasty -- ultrasonic or otherwise. Traditional osteotomies can narrow your nose and would not require an ultrasonic device. While your dorsum does not appear to have a significant hump, I would suggest a tip rhinoplasty where the nasal tip cartilage is modified for a more refined, feminine look as stated in your question. Specifically, I would suggest nasal tip deprojection and slight rotation. The modification of your nasal cartilage can be done with trimming, stitching or grafting. If your septum is deviated and you have breathing difficulty, a septoplasty may benefit you as well. The next step for you is to find the right aesthetic plastic surgeon. In your rhinoplasty consultation, be sure your surgeon addresses all your aesthetic goals and concerns specifically, and discusses the surgery details including the position and size of the incisions used (with actual photographs and not just digital animations), the risks and benefits of the procedure, recovery time, and post-operative course. Also, be sure to see plenty of before-and-after pictures in women who have a similar nose appearance to yours, and preferably at different time frames (3 weeks out, 3 months out, etc) to more accurately predict your own surgical outcome. After seeking out a board-certified plastic surgeon who specializes in aesthetic plastic surgery, I think the most important component of the consultation is to establish a good connection with your plastic surgeon. It’s the connection that allows you to clearly define your desired nose outcome goals, the first step of this whole process. If you don’t feel you have a good connection with your plastic surgeon after your consultation, he/she may not be the best surgeon for you. Answer: Rhinoplasty candidate You seem to be a candidate for rhinoplasty. There is no specific technique that would give you superior result (preservation rhinoplasty may be something you had in mind?), but an experienced, board certified plastic surgeon, expert in rhinoplasty has to decided what and how. In person evaluation and discussion about your goals is recommended.

Answer: Would an ultrasonic rhinoplasty help me achieve a more narrow nose with smaller nostrils? Having performed several thousand successful rhinoplasties, based on your photo you appear anatomically to be a good candidate for an artistically performed rhinoplasty through either a closed or open technique. You can achieve the same result with or without an ultrasonic technique. Keep in mind that following advice from a surgeon on this or any other website who proposes to tell you whether you are an appropriate candidate for surgery and exactly what to do without examining you, physically feeling your tissue, assessing your desired outcome, taking a full medical history and discussing the pros and cons of the operative procedure would not be in your best interest. Natural appearing results need to be individualized and what is appropriate for one patient is not necessarily the best for someone else. I would suggest that you find a surgeon certified by the American Board of American Plastic Surgery and one who is ideally a member of The American Society for Aesthetic Plastic Surgery (ASAPS) or the Canadian Society for Aesthetic Plastic Surgery (CSAPS) that you trust and are comfortable with. You should discuss your concerns with that surgeon in person. Robert Singer, M.D., FACS La Jolla, California
