From the photos, it appears that the predominant problem is one of large dimpled cellulite. In my experience, most devices for treating cellulite, including lasers, have proven of questionable value, with results being subtle, inconsistent and variable and thus pretty much a waste of money. Cellfina is essentially a bells and whistles subcision device, and rigorous science and to my knowledge to date no head to head large scale trials involving significant numbers of patients exist to prove that it is really better than ordinary, relatively quick and inexpensive manually performed subcision (with or without the addition of a biostimulatory volumizing filler). Subcision, using appropriately sized trochars, needles and cannulas, on the other hand, has a track record of being used for over two decades to treat dimpled cellulite. When dealing with the wavy form of cellulite, body vectoring, a variation of a skin boostering injection technique using biostimulatory fillers, may be used to tighten and smooth the skin . There are many ways to deal with cellulite both surgical and non-surgical.On the non-surgical front, we have two technologies that we can use - either the VelaSmooth or SmoothShapes device. Dr. Kulick published peer review papers regarding the effectiveness of these two devices. The SmoothShapes device uses laser energy and mechanical disruption of the fibers that contribute to dimpling. Below is the link to the paper he published in the prestigious journal Plastic and Reconstructive Surgery using an independent pathway VECTRA 3-D analysis. This was the first-time cellulite treatment technology was put to this high-end analysis. The data was not based on doctors showing before and after pictures. There are several ways to address cellulite depending upon depth of cellulite, location, elasticity of overall skin envelope, and subcutaneous fat layer.There is no uniformly successful method for treating it. My practice is now using BodyFX, a non-invasive procedure, that requires no down time to reduce the appearance of cellulite. Cellfina is the most effective treatment I’ve ever used for cellulite. After the application of numbing solution, a device is inserted under the skin and it cuts through the tough, fibrous tissue that causes the cellulite dimples. You can remove up to 30 dimples per hour and studies have shown the results last for three years. This is the longest duration actually clinically proven with any device. Body Fx is another, newer version of external radiofrequency with vacuum massage. These devices in the past have improved cellulite modestly and usually ongoing treatment is needed. In your case, I’d recommend co-treatment with Ultherapy, which tightens skin, and dilutions of Sculptra to round out and smooth your buttocks and thighs. The best treatment for cellulite is Cellfina. However, a combination approach is needed to get the most improvement. Cellulite is an anatomic defect which is usually inherited, where the problems are seen with the thin skin, a tether which pulls the skin inward and tissues under the skin.
