Do you need to break any bones to deproject the nasal tip? Would a tip plasty be a good option in my case? (Photo)

Question

I had my primary rhinoplasty to reduce a dorsal hump and fix my deviated septum, my surgeon did tip refinements to fix my bulbous tip. I was super self conscious about my side profile before and now I am even more because my nose looks longer in my opinion. I'm still early on in my recovery process but I feel so devastated with my results, I don't think it's that much swelling that is making my nose so long, I need advice please.

Sometimes, when a nasal tip is the projected, if the lower bridge needs to be made closer to your face, that can result in bridge widening and necessitate osteotomies. Please consider re-asking your question with a frontal view of your face so we can see the width of your nose. I hope this helps! Sincerely, Dr Joseph

Hello and thank you for your question. Based on your photographs, you may benefit from a revision with a formal tip deprojection. Although osteotomies are not part of the actual tip deprojection process, you will definitely require osteotomies because the dorsum will need to be taken down in order to balance and match the tip deprojection that you need. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person.

In personal exam is essential as well as review of the operative report . Based on limited photos, while it does take time for all the swelling to subside , further improvements can be achieved with Revision Reduction Rhinoplasty  ( in the right hands)--preferably performed through a closed technique.  I suggest that you discuss it with your Operating Surgeon

It's hard to say for sure if you would need to have the bones broken for the deprojection. It'll depend on how far you deproject your nose because that may lead to what we call an 'open roof deformity' that will necessitate osteotomies to close.

So, yes, your nose is "strong".  This can be adjusted.  I'd give it 6 months to see how you do, but, it can be made smaller.
But, what an improvement!  You look so much more balanced and attractive!  It can be hard to go from a large nose to a tiny nose, and sometimes it's easier to make the shape better.
We can improve your nose, yes, if you want. But truly, you look so much nicer!
