Hi, I am 29 years old male to female transgender. I'm planning to have my BA under the muscle to avoid many risks but my surgeon said that he can do subglandular to so he can create good cleavage. If he will do submuscular, it will be too apart from each other and might not look good due to my wide sternum and outward breasts. I want under the muscle because there are less complications, capsular, sagging and ripping. I need help to decide :(
Answer: Implant placement When implants are placed they settle behind the nipple at the breast fold. The sternum is fixed and the width cannot be changed. Placing the implants above the muscle may give the illusion of the implants being a little closer together but they will still be somewhat wide and you'd have to address the nipple position as well.

Answer: Implants You will still have breasts somewhat widely spaced for this is your anatomy, but under the muscle will be the best choice. You should not be as wide as you are now.

Answer: Mini lift with fat transfers or small implants Your breasts are widely separated and you have no cleavage. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. Answer: Breast augmentation You have several anatomical issues now. Your chest wall is curved (which affects how the the implants will slowly descend-more laterally in your case) and your nipples are very lateral. Placing implants medial to get cleavage will accentuate the lateral nipple areola position. Going submuscular limits the medial cleavage while subglandular allows closer medial dissection (better cleavage), this is fraught the potential for medial rippling (the visual appearance of the implant edge).
