Hard bands in breasts 1.5 years post op - what could cause this? (Photo)

I have these hard rope like bands all throughout my breasts, the one pictured is visible due to location. My surgeon thinks it is the acellular dermal matrix that has folded?. My breast often feel tight as if they are being squeezed. I originally had under the muscle silicone implants, due to lower pole deformity on the right from constricted tissue(tubular breast) they were removed and tissue expanders were placed to stretch the constricture. I then had subglandular silicone implants placed with acellular dermal matrix. . Bonus question, if i were to explant/capsulectomy, how difficult is this with ADM integrated? i am 1.5 yrs post op

It sounds incredibly uncomfortable to feel that constant tightness, and your description of "rope-like bands" is actually quite common when Acellular Dermal Matrix (ADM) doesn't integrate smoothly or becomes folded during the healing process. In your case, because you have a history of tubular breast constriction, the ADM was likely used to provide internal support for the lower pole, but if it has curled or scarred down unevenly, it can create those visible ridges and that "squeezing" sensation you're feeling. To answer your bonus question: performing an explant with a capsulectomy when ADM is involved is certainly more complex than a standard removal, but it is very doable for a surgeon experienced in revision work. After 1.5 years, the ADM has likely biologically integrated into your own tissue, meaning it doesn't just "peel off"; it must be carefully dissected out along with the capsule. While this requires a more meticulous surgical technique to ensure all the foreign material and scarred tissue are removed, it is often the only way to truly release that internal tension and restore comfort to your breasts.
