30F. 5’0, 115LBS. Tuberous Breast hx. Had bilateral under muscle implants placed 2015. Original surgeon did not correct the TB deformity however after pregnancy & nursing a few years ago the TB wasn’t as noticeable. HOWEVER, I did have mastitis twice and the breast I had it on seems to have developed CC or silent rupture or waterfall deformity or combo of the 3. I want to get implants replaced however after pregnancy, and covid my dysautonomia is of concern. What would you advise?
Answer: Revision surgery Dear DysautonomiaPrincess, breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery. Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size. While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras. If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals. That can be a drag. Unfortunately, there's no real cure. The best that can be done is to treat the symptoms. There is no literature point to breast implants improve or worsen this phenomenon. This is uncharted territory. The decision to have implants is so going be based resolving your ruptured implant and your desire for improved confidence by achieving breast size, shape, and symmetry. Answer: Revision Your nipples are at different positions. The lower one needs to be elevated higher and the other one needs to be elevated more medial. This can be done through a circumareola approach to reposition the nipple-areola complexes. It may also be possible to undergo replacement of implants in a more medial position to give more cleavage than you have. Best Wishes, Gary Horndeski, M.D.
