Since your current BMI is 44 I would recommend losing about 20-40lbs to reduce your rate of complications during surgery and optimize your results. A tummy tuck can be done later to treat the loose skin and tighten the abdominal muscles. Seek a board certified plastic surgeon who performs many of these procedures, has great reviews and photos, and is an expert in this field.
