Buccal fat removal? I’m a 19 year old female, I’ve never been satisfied with my side profile. I would really appreciate getting some professional opinions on here before traveling to get actual consultations. I had previous bite issues and teeth overcrowding that has been fixed by braces. I see 3 areas listed above which I think could improve my face, but I was wondering which one of those would be the most beneficial to get now, given my appearance & most cost efficient.

Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. You may also benefit from a conservative dorsal hump reduction. A chin implant can be performed at the same time for improved jaw line contour. I always use a medpor implant with screw fixation, which stays in place over time. A silicone chin implant has a high rate of malrotation over time. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. But if a patient has a surgeon change facial features that didn’t bother the patient, s/he may not be satisfied with the result. In your case, based on the pictures, a chin implant can be considered if you are bothered by your chin. Fat augmentation is less predictable but has the potential to add more volume. Similarly, there are minor changes that could be made to your nose, but only if this bothers you. Sometimes a virtual consult can be a good starting point to gather more information and help analyze facial features. Good luck.

From the photos, it appears that you would do well with a rhinoplasty to decrease the bump and refine the tip combined with a small chin implant to balance the nose. This would change the side profile from a curved shape to more of a straight up and down shape. Be sure to see a rhinoplasty specialist who can do computer imaging at the time of the consult.

It is good to think about the face in general as oppose to just the nose when considering the side profile as you suggest. The photos are helpful although there is not an absolute side profile shot so the following comments are just a generalisation of patients with similar concerns to yours. I look at the side profile from the top down. The first thing is that there may be benefit from a minor cartilage graft to the radix region to soften the take off of the nose from the forehead. Reducing the height of the nose will help bring the nose into proportion with the rest of the face but there is also a case for increasing projection of the chin to aid facial harmony. Sometimes, enough chin augmentation can be achieved with fat grafting, which provides a natural solution (see web reference) although a chin implant will achieve amore projection. These are details that are best discussed in person with a board-certified plastic surgeon. Good luck!

An in-office visit would be required to take a look at complete facial balance. This can be achieved with 3D facial imaging; this is a superb assessment tool to see the physical results prior to surgery. I recommend meeing with a Board-Certified plastic surgeon experienced in rhinoplasty. He or she will accurately examine your face, create a 3D simulation and develop a reasonable plan to tailor your facial shape.
