After weight loss surgery: What to do when BMI is too high?

I am 5'2" and weigh 400 pounds. I already lost 150 pounds and have so much excess skin on my legs I can't walk or lay down properly and sitting hurts. I need surgery to remove it as I constantly have infected sores from my thighs pulling at each other. Several surgeons said I was above the bmi which I know I am as that's why my doctor won't get government to pay for it but I am willing to pay for it out of my own pocket. How do I find a doctor willing to help?
