The use of a filler could certainly be considered. However, the problems here are two fold. For one, the side wall of the nose, under the best of circumstances, is a highly vascular area prone to the possibility of vascular compromise when fillers are used there. Secondly, since there was prior surgical rhinoplasty, the anatomy of the subsurface region is likely to have been altered and even more importantly, the vasculature in the area was likely further compromised by the surgiery, a very common aftermath to surgical rhinoplasties. Given the foregoing, it would be most prudent to employ subcision--which is an entirely natural technique for breaking up tethering bands of scar collagen that pulls skin down and promotes new native collagen and elastic fiber synthesis in order to elevate and smooth the dimple. Make sure to consult with an experienced, board certified cosmetic dermatologist. Best of luck.

Treating a dent on the tip of the nose: in my opinion the simplest and best way to address a dent on the tip of the nose such as yours is by placing a small Cartlidge graft. I just saw a patient with an exact same dent, who had previously had a ring through the nose at that point. My guess is that there was a sub clinical smoldering infection which destroyed some of the Cartlidge. Did you ever have a ring through your nose in that location?
