My breasts are a decent size and I don't want implants. However I would consider fat grafting if that would work. If it is possible to have fat grafted from one area of my body to fill in the gap between my breasts and this create more cleavage, would the results be permanent, or would this require routine maintenance? Answer: My breasts closer together, or create more cleavage? Yes, you can achieve a better cleavage, but I would not recommend fat grafts to meet this goal. Your implants appear to have been placed too far apart. Some of the pectoralis muscle fibers were insufficiently divided near your sternum to allow the implants to move enough together. This is unfortunately not an uncommon problem. I recommend seeking a plastic surgeon in your area with experience in this procedure. If, upon examination, your plastic surgeon finds that the implant pockets are open too far laterally, he/ she may elect to suture the pocket in that area as well as dividing the pectoralis muscles as mentioned above. A larger portion of my practice is devoted to revisionary breast surgery. You have a fairly straight forward fix with a lateral capsulorrhaphy (suturing the pocket) and medial capsulotomy (cutting the pocket). I have performed this type of surgery hundreds of times with excellent outcomes. You can read my paper on Capsulorrhaphy on my website. An in person consult with a board certified plastic surgeon could give you a better idea on how this would work. Generally more fat then needed is transferred to the breast knowing some will die, and in end give you the perfect size and desired result. The great thing about fat transfer is it also improves another area of the body. For instance a slimmer waistline if that is the desired harvest area.
