I will be coming up on 5 years since my first rhinoplasty. I've been really unhappy with the results and gave it plenty of time to heal. I want a revision done (by another doctor), but I contacted my original surgeon for my post-op pictures, but nobody at his office is being cooperative/keep getting ignored. Now what? What if he never gets back to me or what if he deleted them?

You need a consultation. Most likely you are opening the eustachian tube that goes from the internal nasal area to the inner ear. When you blow your nose the increased pressure may open the tube resulting in a popping sensation in one or both ears. See a PS or ENT doc for an exam.
I would not recommend any possible irritation of the skin either on the face or directly on the nose, less than one month prior to surgery. The reason for this is that you want the skin to be without any irritations or pimples that might harbor infection that could affect the operation. Gentle...
There should be no reason not to resume skin once the cast comes off as long as you handle the nose itself gently. You might want to avoid the nose itself with strong exfoliants and perhaps RetinA for 2-3 weeks.
