Will OHIP cover needed explant surgery?

Much depends upon the reason for getting your original breast implants: after a mastectomy or some other "medical necessity". If the original reason was purely cosmetic, the insurance is unlikely to cover removal of the implants.
Read your OHIP certificate of coverage. Determine what medical information is required for your medical provider to issue a pre-authorization or predetermination of benefits for breast implant removal surgery. Some medical insurance companies will cover the cost of removing ruptured silicone gel breast implants, correcting severe capsular contracture, addressing infections that do not heal or chronic breast pain, and BIA-ALCL. Then seek consultation with a board certified plastic surgeon who can evaluate your medical condition, and help you in completing the paperwork required by OHIP.

it is hard to tell for sure without an examination. Generally speaking, implant removal surgery is not covered by insurance. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

Unfortunately without knowing the reason for implant placement initially, nor the issue that is currently causing your symptoms, it is difficult to know if your procedure would be covered. I would discuss with your surgeon to get the best advice, or seek second opinions from other surgeons in your area.
