It looks like you had a peri-areolar lift in the past. If so, there is likely a permanent suture under the areola causing the sensation from the tension. If fact, it looks like your areola is stretching beyond the suture diameter. If so, I would recommend conversion to a formal breast lift (full anchor or lollipop) as they are proper procedures to manage breast ptosis. Thank you for sharing.

If permanent stitches are present, they can be excised. The sensation will not improve. Please discuss this with your surgeon.

I'm sorry to hear of the issues you are experiencing. I encourage you to return to your surgeon for an evaluation of your pain. Further imaging studies such as a mammogram or ultrasound may be needed in order to help diagnose your symptoms.
