Botox is a neuromodulator that works by relaxing muscles that lead to the formation of facial wrinkles, and would not work well in your case. A double chin can be addressed through different treatment options, including Kybella and liposuction. Kybella is an injectable treatment that destroys the submental fat cells under the chin so they can be eliminated by the body. Patients usually need more than one treatment session to achieve their full results. Liposuction is a minimally-invasive procedure that removes fat from under the chin using small tubes known as cannulas. It can be performed using local anesthetic and requires only a few days of downtime. Once the fat is eliminated, it is possible the skin will conform over the new contour. If skin laxity continues to be a problem, we can use a non-surgical skin-tightening treatment like Ultherapy or ThermiTight to put the finishing touches on your new facial profile. Make sure you work with a board-certified dermatologist with extensive experience in facial contouring to ensure an optimal outcome from your treatment.
The double chin problem is best treated with liposuction and skin tightening (Facetite, J-plasma, Thermi, Profound). In some cases where there is excessive skin, a platysmaplasty and neck lift may be required.

Botox and other neurotoxins are used to prevent muscle contractions in areas of your face where you would like to diminish facial wrinkling exacerbated with muscle contraction. Reducing your double chin will need a completely different approach. Liposuction (laser of mechanical), Kybella injections, and face/neck lift surgery in more advanced cases will likely be necessary to treat this area.

Based on your photograph, you may benefit from submental liposuction. This decision should be based on a detailed discussion with equal input from both you and your surgeon. Botox works on muscles only. It relaxes the muscle tone and helps in areas like between the eyebrows, the crows feet and the vertical muscle bands in the neck. In your case there is excess fat and skin. If the fat is located under the skin, which can be determined by an examination, it can be removed by liposuction. Deeper fat under the muscle has to be removed through an incision under the chin. The skin would shrink if you are young and have a good skin tone. You need to be seen to confirm these statements.
Botox can improve the shape of the face and neck if overworked muscles are relaxed with Botox but the skin when redraping is not tightening. In the neck in some patients there loose muscle bands which push down on the skin can be elevated by relaxing them with Botox and then the skin doesn't hang as low as when the muscle hung lower. For horizontal rolls of the neck, if fat deposits are causing this undesired appearance then Botox is not the treatment expected to work, and I have not seen that Botox reduces horizontal creases. In the case of horizontal rolls of the neck, Thermitight and/or liposuction is the best way to improve the shape of the neck. ThermiTight can help tighten the skin and the liposuction reduces the fat that causes the bulges. Thank you for your question and photo. The cause of a double chin is excess fat. I suggest you look into Kybella instead for permanent destruction of your under chin fat. Kybella is the first and only FDA approved injectable that destroys underchin fat permanently. It usually takes two to four treatments that are done eight weeks apart to reach desired results.
