I am interested in Hispanic rhinoplasty. I am 26 years old. I am most unhappy with my side profile- my nose droops, and gets droopier when I smile. I am also unhappy with my front profile- my nose is wide, bulbous, and thick skin. There is also stretched skin at the base when I smile. I'd like to shave down the hump, and add structure so that the front is more appealing and not so flat/stretched/wide looking. What do I need to ask for to achieve the after photo?
Answer: Rhinoplasty to change shape... Based on your photos, you are a great candidate for rhinoplasty. The bridge can be taken down a bit and the droopy tip lifted and strengthened. That will make a much better profile view, and will also make it thinner from the front as well. During the surgery, more support will be given to the tip so it will resist drooping afterwards. Be sure to see a rhinoplasty specialist for consultation.

Having an idea of what you want is helpful. The next steps would be to consult with a facial plastic surgeon who has experience in ethnic rhinoplasty to determine what is feasible. There are different ways to approach rhinoplasty - open vs closed. I prefer the first to be exact with the changes I want to create. The thickness of the skin often encountered in Hispanic rhinoplasty needs finessing. Improvements can certainly be made. Hope that helps!

Answer: Just describe your goals! You don't need to "ask" for anything! Describe your concerns and goals to your surgeon, and they can help guide you with a surgical plan. Based on your pictures and description, I would do an open rhinoplasty with a septal extension graft to put your tip in the ideal place, release and reconstruct your Pitanguy ligament to help alleviate the tension on your tip, refine your tip, and probably reshape your alae with alar base surgery (depending on your goals!). Find a surgeon you like with good training and experience with rhinoplasty, and work with them! PS--Skin care before and after surgery is also very important with thick and oily skin, so that should be a part of the conversation as well.
