Irregularities of the nasal tip that appear several months to years after surgery are typically due to edges of cartilage after rhinoplasty surgery. And most cases, these edges of cartilage are not painful, but they can be. It is difficult to see but it looks like it might just be an irregularity of the cartilage. It could also be a suture but your surgeon would know best. Not sure why it hurts. As long as it’s not red nor inflamed you don’t have to really worry about it. But if it’s bothering you go see another surgeon. Take a copy of your OP note with you. Good luck.
