Flat midface! What treatment would you advise?

Facial sculpting and contouring can be created with well-placed fillers (Voluma, Radiesse, Bellafill) and Sculptra, which would give improvement in facial balance. Fat or implants are alternatives. I recommend getting a formal evaluation with a facial sculpting expert. Best, Dr. Emer

Fillers, fat grafting and implants can all provide visual improvement and increase in volume and projection of the cheekbones/midface. Every patient has a different anatomy with different needs and desires. Photos of course would have been extremely helpful (although not quite as good as an in-person examination that allows for direct palpation and manipulation of the tissues).  However, in general, the midface can be shaped and contoured nonsurgically using injectable volumizing fillers. In my NY office, I would generally opt for Juvederm UltraPlus XC for this purpose, whereas in my Israel satellite facility, where a far greater number of regulatory agency approved volumizers are available, I would choose Princess Volume and Aliaxin SV for this area. You would be wise to consult with a board certified aesthetic physician and make certain to insist upon seeing his/her personal before and after photos. Best of luck. 

Fillers such as Juvederm Voluma is great and can add volume to the cheeks however an in person consultation would be needed to determine the appropriate treatment.

Hello, thanks for your question, cheeks can be augmented with filler whether it be fat, synthetic fillers or implants.  If you require a mild augmentation, I think synthetic filler works nicely, can be done in the office with little discomfort/ downtime.  For more significant volumization, I think fat is better as it does not give the "puffy" look you can get from large amounts of hyaluronic acid fillers.  Cheek implants are also and option.  An in person evaluation would be best to determine what is the best option for you personally.  

  Fillers offered temporary augmentation in the cheeks, but must be repeated every few months to maintain the volume in that area.   Cheek implants offer a permanent augmentation to the flat cheek profile and a flat mid face.  Cheek implants are manufactured in a large array of thicknesses, sizes, and shapes which is best determined at  the time of the consultation and examination.  In our practice, we place cheek implants through an intraoral approach under a brief general anesthesia as an outpatient surgical procedure.  For more information and many before and after examples, please see the link and the video below

The 3 basic techniques to augment the cheeks and improve the contour are the injection of fillers, the injection of fat, or cheek implants (there are many sizes and designs). Consult with a surgeon who uses all approaches so you can select the one that is best for you; there are advantages and disadvantages of each.
