White looking bulge under right eye? Is it muscle, fat or scar tissue? Please help my PS doesn't know (Photo)

First surgery in Jan 2022 - midface lift, upper and lower blepharoplasty with canthoplexy and muscle suspension. Revision surgery in August 2023 - lower blepharoplasty to correct herniated fat that wasn't there prior to surgeries. Removal of herniated fat, septal reset, cantoplexy and muscle suspension. Can anyone give me advice as to what is causing this.

Often the lateral fat pad can be inadequately reduced or additional fat can herniate forward. As with any lower lid surgery, the main risks would be excessive fat removal (hollowness) and lower lid retraction (very rare when done with a transconjunctival approach).

It's possible that you have a small cyst under your skin, and if this is the case, a dilute steroid injection might be useful for eliminating the appearance of the bulge. Generally, a cosmetic dermatologist would be best at handling this. I hope this helps! Sincerely, Dr Joseph

You can definitely get local anesthetic only for some eyelid procedures... it might be a lot for some other eyelid procedures! Local is always safer than general. Fat could help for your case.
