I had a facelift and supposedly, a necklift about two months ago. The surgeon did not through an incision under the chin. After surgery, I felt a hard and sore ‘bunching’ at both sides of my neck, behind ears and slightly below on my neck. This hard tissue that bulged out of neck eventually relaxed a littlebut now it has turned into thick pleats and folds. Is this normal? I’ve never seen this on someone’s facelift before? It looks awful. Is this a sign of a poorly executed necklift/ facelift?

I can't comment on your jowls specifically without seeing a photo, but based on your statement, that alone may eventually be cause for revision. As for the pleating on the lateral neck, there is a good chance this will soften and flatten as your healing progresses. It can be part of the normal healing process, even at 2 months postop. Answer: Unsure The bunching sort of looks like pleating/folding of the skin or possibly it is related to the muscle but it's hard to know for sure without feeling it. I'm going to assume it's skin because you said it seems to have relaxed some. If that is the case, it likely means that the skin was not sutured in the right vector so now it's folding funny.

Answer: It's still early, give it time Hello and thank you for your question. After a facelift, you can remain pretty swollen, and this swelling can take a LONG TIME to resolve, especially if this was after a revision (months to a year or more to fully resolve). Regarding the lack of a chin incision- many surgeons will only make this incision if they have to do a platysmaplasty (suture the medial edges of the platysma muscle together in the neck). This procedure is only done if there is significant visible platysma banding present in the neck prior to surgery. A "necklift" is a component of a generally all facelift or revision facelift operations. Based on the bunching and pulling you have described and pictured, it looks like the tissues (including the neck skin) were indeed pulled tighter during your surgery. Regarding the bunching and pleating you're describing, you are still pretty early post-op after your revision surgery. Over the coming months and even year- the swelling will continue to improve and eventually fully resolve, any bunching should flatten out as the skin relaxes a little (either partially or completely- you've already started to notice this), and the scar will become less noticeable as it remodels into a mature scar. I would recommend that you just continue to observe things for now and keep open communication with your operative surgeon. Any revision surgeries, if even necessary, typically are not done until at least 1 year after the prior surgery to allow for the inflammation and swelling to resolve and the skin to relax.
