Thank you for your question! Just based on the picture, it looks like you would benefit from a combination of liposuction with skin tightening. This is a small procedure that could be done while you are awake with just local anesthesia and one small incision under your chin. The liposuction would permanently remove the fat and the skin tightening, such as with a laser, would tighten the skin to give you a sharper jaw line. I recommend seeing a board certified plastic surgeon in person to discuss your goals and get the optimal results.

There can be conditions causing an enlarged neck with not much fat under the skin because the fat is in the plane under the muscle. I would approach this in 3 ways. Reduce fat pocket in the submental area with 3-4 sessions of kybella, followed by facial volumizing with a dermal filler (this helps to recontour and address volume loss), third, I would recommend an Ultherapy session for mid face/cheeks and submental area to tighten and firm your skin (will also help to offset any skin laxity from loss of fat after kybella). From the photos submitted, it does appear that there is some submental fat that could be removed. In addition, I have found facetite to add significant skin tightening over liposuction alone and would recommend the dual procedure.

This is a very common request, and there are many proposed treatments out there, some with little benefit.  Be cautious about your expectations and be cautious about what you inject or remove.  Fillers and Botox will have no benefit in tightening your neck or jawline. Removing fat especially if you don't have enough will just make your skin looser.  Thread lifts on average last 6-12 month so not a long term treatment, 
I don't know your age but you look to be quite young so a surgical neck lift might  be more surgery than you would want to have. However, this would give you by far the most significant improvement. 

Kybella is good for dissolving the submental fat under the chin area and reducing the double chin while contouring the jawline and making the chin more defined. However, a combination approach is needed to get the most improvement with non-invasive options. I recommend a combination approach with Thermage, Ultherapy, and Exilis. VASER liposuction in combination with ThermiRF is the best alternative for contouring the chin and can be performed with a neck lift, if you're a candidate. For the jawline specifically: Botox in combination with radiofrequency treatments can be used to slim and shape the jawline.I recommend getting a formal evaluation with a cosmetic dermatologist and facial sculpting expert. By the time you spend money on all the non surgical options to address fat, skin tightening, and muscle, you will have spent enough to pay for a neck lift. It’s best to get a personal consultation to see what a neck lift can do for you. I do not believe liposuction alone will achieve your desired results.

Hello and thank you for your question.  Based on your photographs, you may benefit from submental liposuction to improve your jawline contour.  This decision should be based on a detailed discussion with equal input from both you and your surgeon.   Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.

Hello and thank you for the photos and questions. Issues with chins can be complicated and are best evaluated with a Board certified plastic surgeon in your area. If you are unhappy with a recommendation from a surgeon, always seek a second opinion. In our practice, options for chins include a neck lift,CoolSculpting, Kybella, truSculptID and Ulthera. Leen83 I appreciate your concern, and while physical examination will be helpful, it appears from your photos that micro liposuction with neck and submental contouring is by far your best option. I use very small cannulas in the neck which allows me to deftly contour and tighten the neck in this fashion. Your skin should re-drape nicely as it appears you’re still quite young.  Kybella is sometimes useful for small, well defined areas of the submental region, however in your case there is diffuse fat both of the submental and neck regions.
