The main reason that fat injection for the buttocks is a high risk is that it is possible to accidentally inject into the large pelvic veins if the surgeon is not careful to avoid this specifically. This type of risk situation does not exist with breast fat injection, and if it occurred it would likely be a very small volume of fat, and not the life-threatening situation that can occur with buttock injection.
Fat embolism is possible with any fat grafting procedure. Fortunately the incidence of fat embolism is low. Using blunt cannulae, low pressure injection and a knowledge of anatomy help to lessen the risk. Your plastic surgeon can help guide you as to what measures they take to make surgery safe.
