How is permanent damage to alar cartilages diagnosed & fixed in revision?

There could be a few causes: nasal trauma to tip after successful rhinoplasty where Medpor septal graft & ear cartilage help together alars but helped me breath (nose hit hard, pivoted left); bad revision involving removal of both for shield graft that was (mis)placed where ear cartilage was. I was breathing better with the Medpor/ear cartilage, of course, & the nose hadn't collapsed. I want my implant & ear cartilage back anyway, but what if my nasal cartilages were destroyed,and I don't know?
