Are there doctors in the USA that do high volume liposuction (more than 4 liters)?

You need to be evaluated in person. First thing to know is that liposuction is not for weight loss and it only removes localized fat under the skin. Like deflating a balloon. The more the balloon is deflated, the more wrinkly its surface gets: that would be your extra skin that has been stretched. A board certified plastic surgeon can assess if liposuction is a good option for you and then decide on the volume to be safely removed.

Most surgeons will hold the liposuction to 5 liters in the outpatient setting except in Florida where the limit is 4L. You are on the right course of losing weight to bring your BMI of 37.4 closer to 30 to maximize results and reduce the risks of complications during and after surgery. Liposuction is not to be considered a weight loss procedure but a contouring one.

In the US its recommended to stay under 5L, which we do frequently. Although liposuction is not a weight loss procedure you can do numerous treatments plus hormonal optimization for weight loss and get a good results.  You need a consult.

At BMI over 37- a person is considered obese. You should not consider liposuction at this time, instead focus on loosing significant weight, focus on healthier lifestyle, and once you achieve a healthy weight -then re-assess your goals. Liposuction/ liposculpture  is a body contouring operation, not a weight loss operation.

I appreciate your concerns and frustrations, however there is a very good reason why plastic surgeons in the United States do not perform high volume liposuction. This practice causes significant physiological changes in the body and is simply unsafe! This is another reason why cosmetic tourism is a bad idea because safety standards in many other countries, where cosmetic operations are cheaper, do not equal the safety standards of the United States and thus deadly complications can occur.Try to adopt a reasonable diet and exercise program to lose as much weight as possible to lower your BMI below 30. Liposuction, or the suction removal of subcutaneous fat, is a contouring method and not a weight loss technique.  The best results are seen on patients at or near their ideal body weight or for stubborn areas of fat deposits.Liposuction can be performed on more "robust" individuals with the understanding that their clothes will fit better  (but not change size in clothes) and as a jump start to a weight loss program.  There is no real universal agreement on what is considered "high volume" liposuction.  Some states have placed restrictions on the amount of fat that can be removed in one session (Florida and New York), but the majority of states have not.Always consult with a Board Certified Plastic Surgeon who performs body contouring procedures frequently to assure the best possible result.  These procedures should be performed in only nationally accredited surgical facilities that have board certified anesthesia coverage in case of any emergencies.  It is an outpatient procedure with a rapid recovery.Good Luck!

In my opinion high volume liposuction is not 4 liters. The ASPS considers high volume liposuction to be 5 liters. If performed consider performing in a hospital in case overnight stay is necessary. 8 liters is considered the recommended maximum amount of removal in one day. That being said I routinely do 4-6liters of liposuction even with a concurrent abdominoplasty in a hospital setting. Taking four liters from multiple body areas in a relatively thin frame is more stress on the body than the same volume in one or two areas in a large frame. 

High volume is statutorily barred in NY and could result in licensure loss should any complications  arise. That should say it all.
