Doctors, I had 5 years ago a ptosis surgery anterior approach and repeated it 3 years ago ( same technique) last year I had mullerectomy technique as I still wasn't happy with the result. 1 month ago I re-did mullerectomy revision, and now the result is to cry. Its way worse then before and I think I'd need a 5th operation. What technique shall be used and will I be able to make it better?

That is correct, that botox is typically not used in the tear trough area. I love Bellafill in for the lower eye hollows, but I do think that a lower lid blepharoplasty would be the best option based on this photo.
