The “art” in filler therapy is choosing the experienced, artful injector who understands which filler to use where, what layer and how to get the results desired. The “science” in filler use is that they all coalesce into a more firm, ball-like or tubular form and to use dissolvable hyaluronic acid (HA) clear gel fillers if one is not fully secure in achieving the desires results. • I like Radiesse in patients with dark under-eyes and flat or low orbital-cheek bones, catheter injected along the bone. • I use layer HA fillers for volume, as you mentioned like the “apple or cheek bone” areas where projection and fill are needed. • I have found fat to be an excellent volume filler but responds poorly to projection and in the eyelids themselves. A facelift can elevate the tissues lateral to the “mid-face” or apple-cheek area in the direction of the temples. For the descended, hollow, tear trough, lower lid crease and fat bag areas, you will need to consider a Mid-Facelift which can redistribute the fallen cheek tissues and separations of the cheek fat compartments directly upward toward the pupil.
