This could be placed in a dual plane or underneath muscle for the most natural look. The key is, I would not use saline implants, I would not choose "too large" a size. In regards to your back and upper body, I would suggest liposuction. This, of course, is without pictures or a physical examination. However, I believe with upper back liposuction and possibly mid back liposuction, you would obtain reduction in the size of your upper back.
