Hello and thank you for your question. You appear to have nostril asymmetry and medial crural footplate asymmetry. Based on your photographs, you may benefit from a tip refining rhinoplasty, columellar strut graft, suturing your medial crural footplates, and alar contour grafts. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. Nostril symmetry is one of the hardest aspects to achieve in rhinoplasty. Alar base reduction can correct width or flare asymmetries which affect the sides of the nostril. The columella and alar rim are affected by the tip cartilages themselves. Your tip is leaning to the right, which is the primary cause of the nostril asymmetry. Correcting the tip is a critical step for you. This may then result in a nostril attachment asymmetry which can be partially or totally corrected with an alar base reduction.
