What's next after microneedling for PIH skin?

Question

I just did Collagen PIN microneedling 5 days ago. (Previously 2 fraxels 2 years ago which ruined my skin IMO..) I have peeled and I see pore size and superficial texture issue improvements. What's next for my easily hyperpigmented skin? I'm doing Elta MD sunscreen but how can I help correct my PIH scars during the summer without hydroquinone and safely? No derms near me understand my PIH issues I have even with zero sun exposure...ideas? Thanks!

Dark scars can be problematic as they can be visible far longer than one might expect. The fundamental problem with dark scars is a process known as post-inflammatory hyperpigmentation in which the pigment cells near the scar are stimulated. In clinical practice, we always begin with application of Melarase AM and Melarase PM creams (coupled with Melapads) to help smooth out the color and pigmentation. Once the creams are started, lasers can be used to further reduce the scarring.

Thank you for your question and posting pictures.  We've been using the PicoSure FOCUS treatment for 2 and half years with great success.  It does require a series of treatment, usually a minimum of 4.  Depending on the depth of your pigment, you may need several more treatments.  If the pigment is not moving at all after 2-3 treatments, we sometimes will treat patients with the PicoSure Flat Lens.  You may not be a candidate based on your skin type.  You might contact the office in advance of your next appointment to discuss your concerns.  Best of luck.

Firstly we need a diagnosis before treatment - yes this could be PIH or melamsa, both behave differently. You do need to see a dermatologist for advice and a close examination. If this is melasma, QSL or even Picosure lasers can do wonders. It does take 5- 10 sessions for gentle QSL lasers or 3-4 of pico lasers with bleaching creams. You must be sun smart. As for microneedling, I really don't know if its is the correct procedure to repair your skin condition. A close examination will be needed. All the best, Dr Davin Lim. Brisbane Australia.
