Pain after ear surgery 7 years go - could be caused by permanent suture?

Hello, In 2015 i've had Ear Surgery done and since then i've irregularly been experiencing a sharp temporarily pain in my left ear, that comes and goes. During the pandemic i've noticed this more frequently due to wearing a face mask. My doctor says it's caused by a permanent suture, but is that true? It doesn't hurt when i touch my ear. And using scar cream made the pain less. Could it be caused by scar tissue or something else? And what are the risks of having a permanent sulture removed?
