I had my rhinoplasty a year ago. The surgeon said that my nose was not simple but also not too difficult, thin skin on the bridge and thick skin on the tip. I told him I would rather have him take a little bit than too much, because I wanted a natural result. As the swelling went down the indent on the nostril became worse and more visible. Can this be fixed and how? I would like to hear your opinions. White eyes are 6 months post op and 1 year post op.

Nose irregularities such as indents can be a result of a variety of causes, including the wrong technique in shaping the nose, causing weakening of the cartilage and accumulation of scar tissue. To know for sure, I recommend scheduling an in-person consultation with a reputable board-certified facial plastic surgeon, particularly one with a strong rhinoplasty portfolio. Your surgeon can do an in-depth evaluation and may recommend fillers or revision rhinoplasty to address your concerns. Only a skilled and experienced surgeon with a broad understanding of the complexity of the nasal structure can deliver the best possible outcome.

Most surgeons prefer waiting approximately a year for resolution of scar tissue before performing a revision. Check with your own surgeon and see what he thinks, since he knows what was done in the tip area and is most familiar with your tip anatomy.

Answer: Tip revision rhinoplasty Thank you for enquiry and sharing your photographs. I am sorry that you are experiencing difficulties after your operation. In the first instance, I would strongly advise you to discuss your concerns with your surgeon. The surgeon understands more accurately the structural arrangement in your nose and may be able to advise regarding a simpler solution. The external appearance suggests that the position of the left alar cartilage is more sunken in which could be due to scar tissue developing inside the nostril or by weakness in the cartilage left after reshaping and refining the tip. This can be addressed with reinforcement of the left alar cartilage. If there is scar tissue inside the left nostril that produces tension, it would need to be released and possibly prevented from coming back with a full-thickness graft. In most instances reinforcement of the cartilage is sufficient. Another point to consider is that when the contour on the left side becomes fuller you may find that the overall appearance of the nasal tip is slightly too wide. As a result it may be necessary to reduce slightly on the right side when building on the left in order to produce a more balanced appearance.

This is when the sides of the nostril collapse in words giving the nose an appearance as if was pinched . This deformity is often the result of the surgeon having taken too much cartilage from the area which leads to weakening of the nasal tip and inwords displacement of the nostrils. It can be corrected with another operation that adds cartilage to the sides to strengthen the area and make it come out again.
