Question

I am interested in a revision rhinoplasty to achieve a more defined, lifted nasal tip and a shorter profile (a 'semi-Barbie' look). Given that my initial rhinoplasty was performed 18 years ago and my current results are still quite good, what is the likelihood of achieving this specific outcome?

While the photos are limited—especially without a clear profile view—it sounds like you’ve had a long-lasting, successful result. If you’re still generally happy with your nose, it’s worth approaching revision surgery cautiously. Small refinements are possible, but revision rhinoplasty is more complex, and pursuing subtle “perfection” can sometimes lead to unintended trade-offs or dissatisfaction.

Hello and thank you for your question. Based on your photographs, you may benefit from a revision rhinoplasty. A surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Fascial grafts may also be used to help improve tip refinement. This can also be designed to improve nostril symmetry. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
